Community Care Assistant duties include:
- Assist in the development of the philosophy, goals and objectives for the care practice.
- Assist in the assessment of the effectiveness of care implementation and delivery
- Implement action to meet and maintain care standards.
- Encourage a model of self-care and Service User rehabilitation.
- Record relevant activities in Care Plans
- Encourage innovative methods for the delivery of care
- Encourage a model of self-care and Service User rehabilitation
- Work in cooperation with members of multi-disciplinary health teams in order to maximize opportunities for Service User therapeutic care

- The successful candidate will have a minimum of at least 6 months experience of caring in the community
- Have eligibility to work within the UK
- Availability to work at least one in every three weekends
- Be able to provide 2 references
- Flexible and reliable
- Have excellent communication skills
- Willing to travel
- Have a full driving licence and be a car owner .

This job is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975.
